CHC MISSION:
Since 1982, the California Health Collaborative (CHC) has a long history of working with youth and their families through culturally responsive and community-centered public health programming. With a mission of “changing lives by improving health and wellness", CHC is committed to addressing and advocating for health justice among California’s underserved and underrepresented communities who have limited access to resources, and often face barriers related to culture, language, income, race, education, gender, geography, or immigration status.
JOB SUMMARY:
The California Health Collaborative is seeking a Community Engagement Coordinator (CEC) to take a leadership role in the Asian Pacific Islander Partners and Advocates Countering Tobacco (API PACT) Project. With support from the Director of Program Services and Project Coordinator will lead the team in the engagement of youth and planning of youth activities for the API PACT Project. API PACT is a Central Valley-based tobacco prevention project serving Fresno and Merced County Asian American, Native Hawaiians, Pacific Islander (AANHPI) communities with intentional community engagement that centers voice, agency, wellness, culture, and healing as we build momentum towards ending California’s tobacco epidemic. The program objectives for 2025-2028 include working closely with local AANHPI residents to adopt and implement a smoke-free outdoor public places and multi-unit housing policy in the communities of Clovis and Livingston and partner with AANHPI advocates to advance all areas of health equity in their communities.
ESSENTIAL FUNCTIONS:
  1. Consults with the API PACT Leadership Team (Project Director and Project Coordinator) relative to planning, policy, and program direction.
  2. Presents and models values and behavior consistent with inclusivity, respect, and affirmation of all partner identities.
  3. Responsible for providing intervention activities to reduce/prevent the use of tobacco and reduce tobacco-related health disparities among the AANHPI community. The CEC, in conjunction with the Project Director and Project Coordinator will represent the program at all community and educational-based community events and activities with particular focus in the communities of Livingston and/or Clovis.
  4. Lead and facilitate the Community Engagement Agreements (CEA) Program annually with 1-2 local organizations serving the AANHPI population whose projects support the objective and assist in moving program efforts forward. The CEC will prepare and plan a process for recruiting and interviewing agencies for the subcontract.
  5. Develop paid media and social media advertisements and maintain the program’s Facebook and Instagram social media pages to share educational information, cessation resources, and program highlights and updates.
  6. With the support of the Leadership Team, assist with the coordination, development and maintenance of a program website that will act as a central hub for networking, strategizing, and information dissemination.
  7. Assists with planning and logistics for all program meetings, educational presentations, and community events.
  8. Builds intentional, supportive relationships with community advocates and partners by building support for people power and voice in program and community decisions. This includes actively stepping back to allow the community partners and youth to step forward in program decision making, elevating their voices and creating space for youth and all other partners in community and program decisions.
  9. Supports in reducing barriers for youth participation by identifying methods to meet youth where they are at, identifying modes of transportation to support youth in accessing program activities, and on occasion transports youth in personal vehicles.
  10. Be available to meet with youth and adult program participants during the school day, after school or on weekends or evenings as required to meet the needs of the community.
  11. Ensure community advocates are central to the planning of all program activities and ensure authentic engagement in the project’s efforts in Livingston and/or Clovis.
  12. Assists with the development of youth coalitions tasked with leading Policy, Systems and Environmental change in Livingston and/or Clovis regarding a smoke-free areas and multi-unit housing.
  13. Take a lead role in developing ongoing trainings for youth coalition members, adult program participants, and interns on topics related to: Policy and Systems Change, Telling your Personal Story, Stigma-Free Tobacco Prevention, Healing Centered Engagement, Working with Elected Officials, and Advocacy.
  14. Assist with ongoing evaluation activities and support in preparing biannual reports of the impacts of the project.
  15. Represents the California Health Collaborative at meetings with committees, community agencies, care providers and related entities, and facilitates inter-agency linkages, communication, and collaboration.
  16. Performs other duties as assigned.

REQUIRED EDUCTION AND/OR EXPERIENCE:
Education and experience requirements are a bachelor’s degree in Community Health, Child Development, Social Work or a related field and a minimum of one year experience working with diverse youth and families; OR A combination of work experience and education, indicating an ability to fulfill the requirements of the position. Cross-Program Collaboration
Collaborate across CHC programs to increase awareness of CHC services, promote CHC-branded programs and standardized referral pathways, and consistently utilize CHC’s online referral asset (including the CHW Navigation hub and related support programs) to strengthen access, coordination, and continuity of care.
External Awareness amp; Representation
Proactively promote CHC priorities, programs, and referral assets to all external stakeholders—including enrollees, partner organizations, and community entities—to eliminate fragmentation, prevent missed referral opportunities, and ensure consistent awareness and utilization of CHC services.
Professional Standards and Virtual Meeting Expectations

Camera Usage Requirement
