  • Master's Degree in Counseling or Psychology
  • Current LPC, LAC, LMFT licensure
  • Ability to complete comprehensive psychosocial assessments and coordinate care effectively
  • Knowledge of HIPAA regulations, patient safety standards, and regulatory compliance requirements

What You Will Do:

Performs professional social work to facilitate the movement of patients through the continuum of care while ensuring their biopsychosocial needs are met, appropriate to age and clinical population served.

  • Conduct comprehensive psychosocial assessments, identifying current problems and contributing factors
  • Provide therapeutic interventions including individual, family, and group sessions as ordered
  • Identify and respond to abuse, neglect, and safety risks (including suicidal or homicidal ideation) in accordance with policy
  • Collaborate with interdisciplinary teams to develop and update patient care plans
  • Communicate effectively with patients, families, and healthcare team members to support informed decision-making
  • Monitor patient progress, reassess needs, and update treatment and discharge plans accordingly
  • Facilitate discharge planning by coordinating community resources and referrals
  • Ensure accurate documentation and compliance with regulatory, legal, and organizational standards
  • Support patient education and engagement throughout the care continuum
  • Promote patient safety, confidentiality, and adherence to HIPAA guidelines
