Revolution Healthcare Services is seeking a travel nurse RN Case Management for a travel nursing job in Santa Rosa, California.

Job Opportunity – Case Management RN – Northern California

Start Date: ASAP

Location: Santa Rosa, California

Compensation: Details provided to qualified applicants

Schedule: Full-time assignment with weekend rotation of 2 weekends every 6 weeks; no call required; patient caseload of approximately 20 patients

Highlights:

  • Utilize Epic charting system in a fully computerized documentation environment
  • 40-hour orientation provided to ensure successful integration into the care team

Requirements:

  • Active and unencumbered RN license
  • Minimum 2 years of case management experience required
  • Current work history within the past 2 years (hard requirement)
  • BLS certification required
  • Epic charting system experience required
  • Previous travel experience preferred
  • Community hospital, LTAC, and trauma center experience preferred
  • Proficiency in discharge planning, care coordination, utilization of InterQual criteria, and determining medical necessity per evidence-based guidelines
  • Experience arranging DME, SNF placements, and ambulance transport independently
  • Knowledge of HMOs, IPAs, and insurance authorization processes
  • Familiarity with CMS guidelines, HIPAA regulations, and The Joint Commission standards
