PRN/Per Diem/Flex RN Needed for Private Duty In Home Senior Care
Touching Hearts at Home is seeking PRN/Per Diem RN's who are looking for that perfect part time/flex job. The main role of this position will be to support our Clinical Operations team under the direction of our Director of Nursing. For this role you will be paid a flat fee per task assigned/completed and mileage. We can work with as little as a few hours a week of time available to up to 20 or more hours available. This is the perfect flex role for an RN and we'd love to discuss this more in detail so please let us know you are interested to learn more.
Touching Hearts at Home is a private duty home care provider that specializes in senior care. We serve our clients in their homes or place of residence all across the Twin Cities metro area. Our home office is centrally located in Bloomington and we provide full PPE gear to our staff and adhere to the MDH Covid guidelines.
Job Responsibilities Include:
-New client RN assessments
-Client change in condition visits/followups
-Enrolling clients in to our Medication Management Program
-Ongoing Medication Management visits
-Current client and caregiver supervisory visits
-Delegated task training and supervision
-Specific skilled nursing tasks
-After hours/weekend on call
Job requirements:

  • Current Minnesota licensed RN
  • Submit weekly availability
  • Experience with seniors/in home care preferred, a "heart" for seniors
  • Team player, crucial
  • Excellent customer service and assessment skills
  • Experience with overall care coordination.
  • Be willing to go that "extra mile".
  • Excellent communications relationship-building
