Job Description
Location: Onsite Mon-Fri 5 Days a week in our CVFP New London Office (1088 London Links Dr, Forest, VA 24551)
Privia is looking for a forward thinking, organized, energetic Nurse Care Manager that delivers on objectives and seeks an opportunity to develop, implement, and deliver change within our medical group and affiliated partners. Working closely with the population health teams, networked facility partners, primary care providers, and practice operations at Privia Health, the Nurse Care Manager will provide extensive care coordination for Privia's highest risk patients.
  • Manage a diverse caseload of patients, ensuring timely and efficient follow-up on care plans, appointments, and treatment outcomes to optimize patient health and satisfaction.
  • Build trust and rapport with a select group of Privia patients to reduce high-cost utilization and enhance clinical outcomes through telephonic and in-person engagement.
  • Serve as a liaison between primary care providers (PCPs) and patients, assisting them in navigating and accessing appropriate care resources.
  • Conduct holistic assessments that encompass medical, social, economic, and behavioral factors to identify opportunities and barriers for improving outcomes, collaborating with patients and care center teams to develop comprehensive care plans.
  • Perform Annual Wellness Visits for our Medicare population at assigned locations, including assessing vital signs, administering vaccines, and reviewing medical histories, medications, and preventive healthcare needs.
  • Demonstrate a strong foundation in chronic disease management and apply clinical knowledge and critical thinking to educate patients, support collaborative goal setting, and implement care strategies that reduce high-cost utilization and improve long-term health outcomes.
  • Ensure all assessments and care planning tasks are completed by an RN or LPN under RN supervision, adhering to established protocols and standing orders.
  • Utilize Athena and Privia applications to identify high-value opportunities for reducing utilization and closing quality gaps.
  • Maintain a courteous and supportive demeanor when interacting with patients, physicians, and colleagues, and participate in special projects as needed.
  • Other duties as assigned.

Qualifications
  • BS in Nursing (or equivalent) preferred and at least 3+ years of case management or care coordination experience.
  • Active unrestricted license in VA
  • Strong computer and EHR skills and expertise
  • Must comply with all HIPAA rules and regulations

The salary range for this role is $64,800.00 to $72,000.00 in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs). This role is also eligible for an annual bonus targeted at 10% .The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
