Job description

Job Title

Responsible for the direct medical care of THIRA Health patients through regular patient check-ins. Coordinate care with clinical treatment and assist in treatment plans for patients.

Required Qualifications
  • Requires associate's degree in nursing (ADN); bachelor's degree preferred (BSN)
  • Must possess and maintain licensure of either RN; must maintain good standing with the State Licensing Board
  • Demonstrated proficiency in the use of Microsoft Office and the ability to use various software programs
  • Must be able to successfully pass a comprehensive criminal background check (Federal and state)
  • AFH Staff: ability to successfully pass the Department of Social and Health Services background check
  • For positions involving driving personal vehicles or company vehicles as part of the job duties, valid driver's license, proof of personal liability insurance
  • AFH or other assigned positions – successfully pass THIRA's Driver Assessment
  • Ability to work evenings, nights, weekends & holidays as needed
  • Perform other duties as assigned
Preferred Qualifications
  • Minimum one (1) year prior experience as a nurse or C.N.A. required; prior mental or behavioral health nursing experience highly preferred
Essential Job Functions and Responsibilities

  • Troubleshoot problems/situations that arise with patients within the facility; provide milieu support and general coverage for programs as needed; on-call on the floor to meet the needs of the clinic
  • Take vitals on PHP/RTF patients as required document accordingly in patient's chart
  • Perform initial new patient medical assessment, including but not limited to, reconcile medications, obtain medical/surgical history and most recent labs, confirm all patient allergies, and document all findings in patient chart
  • Maintain patient illness tracking log and consult with patients regarding questions/concerns about presenting physical medical symptoms
  • Coordinate care with clinical treatment team and assist in treatment plans for patients as necessary
  • Maintain all medical data for patients in EMR patient charts
  • Support Nurse Manager & Infection Control Team with staff illness record maintenance as needed
  • Design and lead health and wellness lectures/groups for patients
  • Assist PHP/RTF patients with self-administered medications if needed and maintain the medication self-administration records and orders
  • Support prescribers with creation of lab and EKG orders for all patients as necessary, follow up with providers when needed
  • Perform skin assessments for self-harm behavior, treat as needed and refer to outside providers when necessary
  • Communicate with outside providers and patient family members regarding medical information as directed
  • Assist with medical supply ordering and inventory; maintain medical budget
  • Coordinate disposal of biohazard waste with appropriate vendors as directed
  • All other duties as assigned

Working Environment

Work is performed primarily indoors, with both artificial and natural lighting. Equipment includes standard office equipment, i.e. computer, telephone, fax machine, copier, printer, and scanner. Physical demands include sitting, walking, standing, bending over, repetitive use of hands, and lifting up to 25lbs. Position requires close contact with all THIRA Health staff and mental health patients. Hours and days can vary within a Sunday through Saturday week.
