Home Health RN Responsibilities:

  • Conductinitial and ongoing patient assessments in the home setting
  • Developand update individualized care plans in collaboration with the care team
  • Administermedications, treatments, and injections as prescribed
  • Performwound care and dressing changes and other complex care
  • Monitorvital signs and symptoms, reporting changes to the physician
  • Educatepatients and caregivers on disease processes, medications, and self-care
  • Coordinatecare with physicians, therapists, and other healthcare professionals
  • Documentall visits and care provided in the EMR
  • Ensurecompliance with agency policies, Medicare/Medicaid, and other regulations
  • Participatein case conferences and interdisciplinary team meetings
  • Provideemotional support and promote patient independence

Requirements:

  • Washington RN licensure
  • Home Health/Hospice agency experience preferred, but not required
  • Current BLS certification
  • Valid driver's license and insured vehicle
