Position Summary

We are seeking a Clinical Pharmacist who will focus primarily on patient interactions via phone. This role emphasizes medication counseling, education, and ongoing clinical support to ensure patients understand their therapy, adhere to treatment plans, and have a positive experience with our pharmacy services. The Clinical Pharmacist will collaborate with prescribers, technicians, and support teams to deliver safe, accurate, and empathetic care.


Key ResponsibilitiesPatient Counseling & Phone Support
  • Conduct daily phone consultations with patients regarding their prescriptions, medication regimens, side effects, and administration techniques.
  • Provide clear, empathetic education tailored to each patient's needs, including adherence support and lifestyle guidance when appropriate.
  • Answer patient questions related to therapy, drug interactions, and monitoring requirements.
  • Document all counseling sessions and clinical interventions in compliance with regulatory and organizational policies.
Clinical Oversight & Safety
  • Verify prescriptions for clinical appropriateness, accuracy, and safety.
  • Monitor and resolve medication-related problems, collaborating with prescribers when therapy modifications are needed.
  • Identify potential adverse drug reactions and ensure timely interventions.
  • Support adherence and persistence programs to improve patient outcomes.
Collaboration & Communication
  • Work closely with clinical pharmacy technicians, call center agents, and support teams to triage calls and escalate clinical questions appropriately.
  • Communicate with prescribers and healthcare teams to clarify orders, address patient concerns, and ensure continuity of care.
  • Participate in interdisciplinary team discussions to optimize patient therapy.
Compliance & Quality
  • Maintain compliance with federal, state, and organizational regulations, including HIPAA and patient privacy requirements.
  • Ensure counseling is aligned with best practices and clinical guidelines.
  • Participate in continuous improvement initiatives related to patient support and pharmacy services.
Qualifications
  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree required.
  • Active Ohio pharmacist license preferred and in good standing or willing to obtain within 180 days
  • 1-3+ years of direct patient care experience preferred (retail, specialty, or ambulatory care settings).
  • Strong knowledge of pharmacotherapy, medication safety, and patient counseling techniques.
  • Excellent verbal communication and active listening skills.
  • Compassionate, patient-centered approach to care.
  • Comfort with technology platforms, including pharmacy management systems, call routing systems, and electronic health records.

  • Full-time, with flexibility for evenings or weekends depending on patient and business needs.
Essential Functions of the Job
  • Must be able to answer phone calls and perform patient counseling duties throughout a standard 8-hour workday.
  • Must be able to remain seated and engaged at a workstation for extended periods while maintaining focus on patient interactions.
  • Must be able to use computer systems, telephony platforms, and electronic health records efficiently to complete daily tasks.
  • Must have reliable internet access and a private, quiet workspace to ensure patient privacy and effective communication.
  • Must be able to meet productivity and quality standards for remote clinical and patient support operations.
