Hospital Pharmacist (Contract) โ€“ Day/Evening Rotating Shifts

Location: Glenwood Springs, Colorado
Facility: Valley View Hospital
Job Type: Contract Assignment
Start Date: ASAP
Assignment Length: Approximately 20 Weeks
Schedule: 40 Hours Per Week | Rotating Day/Evening Shifts | Rotating Weekends

Position Overview

Valley View Hospital is seeking an experienced Hospital Pharmacist to join its pharmacy team on a contract basis. The ideal candidate will possess strong clinical pharmacy experience, proficiency with Epic, and an active Colorado Pharmacist License. This position offers the opportunity to work in a collaborative healthcare environment focused on delivering safe, effective, and patient-centered pharmaceutical care.

Schedule Details
  • Rotating day and evening shifts
  • 40 hours per week guaranteed
  • Rotating weekends (7:00 AM โ€“ 12:00 PM)
  • Occasional overnight shifts may be required based on departmental needs
  • Holiday coverage required
Key Responsibilities
  • Interpret, verify, and process medication orders accurately
  • Maintain computerized patient medication profiles
  • Prepare and dispense medications, including IV admixtures, chemotherapy agents, and parenteral nutrition products
  • Monitor medication therapies for safety, efficacy, contraindications, and drug interactions
  • Perform pharmacokinetic dosing in accordance with hospital protocols
  • Review culture and sensitivity data and provide antimicrobial stewardship recommendations
  • Evaluate patient charts and clinical data to support optimal medication management
  • Detect, document, and report adverse drug reactions
  • Provide clinical consultations and medication recommendations to physicians and healthcare staff
  • Educate patients and families regarding medication therapies
  • Document clinical interventions and pharmacy activities accurately
  • Participate in quality improvement initiatives and departmental audits
  • Assist with staff education, orientation, and training programs
  • Serve as a member of the Code Response Team
  • Maintain compliance with all federal, state, and local regulations
Required Qualifications
  • Active Colorado Pharmacist License in good standing (required at time of submission)
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or equivalent
  • Minimum of 3 years of pharmacist experience
  • Epic experience required
  • Current BLS Certification (American Heart Association)
  • Current ACLS Certification (American Heart Association)
