Position Overview
Valley View Hospital is seeking an experienced Hospital Pharmacist to join its pharmacy team on a contract basis. The ideal candidate will possess strong clinical pharmacy experience, proficiency with Epic, and an active Colorado Pharmacist License. This position offers the opportunity to work in a collaborative healthcare environment focused on delivering safe, effective, and patient-centered pharmaceutical care.
Schedule Details
- Rotating day and evening shifts
- 40 hours per week guaranteed
- Rotating weekends (7:00 AM โ 12:00 PM)
- Occasional overnight shifts may be required based on departmental needs
- Holiday coverage required
Key Responsibilities
- Interpret, verify, and process medication orders accurately
- Maintain computerized patient medication profiles
- Prepare and dispense medications, including IV admixtures, chemotherapy agents, and parenteral nutrition products
- Monitor medication therapies for safety, efficacy, contraindications, and drug interactions
- Perform pharmacokinetic dosing in accordance with hospital protocols
- Review culture and sensitivity data and provide antimicrobial stewardship recommendations
- Evaluate patient charts and clinical data to support optimal medication management
- Detect, document, and report adverse drug reactions
- Provide clinical consultations and medication recommendations to physicians and healthcare staff
- Educate patients and families regarding medication therapies
- Document clinical interventions and pharmacy activities accurately
- Participate in quality improvement initiatives and departmental audits
- Assist with staff education, orientation, and training programs
- Serve as a member of the Code Response Team
- Maintain compliance with all federal, state, and local regulations
Required Qualifications
- Active Colorado Pharmacist License in good standing (required at time of submission)
- Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or equivalent
- Minimum of 3 years of pharmacist experience
- Epic experience required
- Current BLS Certification (American Heart Association)
- Current ACLS Certification (American Heart Association)
