Job description

Position Overview:

The Per Diem Registered Nurse will focus on client admissions, care plan development, and modification based on consumer needs. They will assess patient conditions, identify risks, and implement appropriate care plans. The nurse will oversee and coordinate skilled care in the consumer's home and community, ensuring quality and safety. They will provide individualized services in alignment with the consumer's living philosophy, building strong therapeutic relationships with patients and families. Upholding OSH's mission, vision, and values is crucial to the delivery of care services.

Essential Job Functions:

  • Performs Start of Care, Resumption of Care, Supervisory visits, as needed.
  • Performs 60 day consumer re-assessments and post-fall assessments, as needed.
  • Ensures that the professional standards of community nursing practice are followed.
  • Provides and ensures consumer care services under the direction of the Director of Nursing and per the physician's order for the plan of care, which may include simple wound care and tube feeding.
  • Adheres to policies for, and keeps records of criteria for admission, and to service case assignments and case management.
  • Works in conjunction with other health care professionals in the community and families to identify resources available and to ensure access of information to consumers.
  • Provides support and education to consumers and families related to appropriate and available health care resources.
  • Ensures accuracy, completeness, and timeliness of documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
  • Reports to team and support staff in identifying Agency/consumer changes in condition and needs and opportunities for quality improvement.
  • Regularly checks in with the clinical team to alert the clinical team of any concerns, changes, or other situations that arise during their visits that would require the clinical team's attention. (Examples include emergent situations, wound findings, falls, ER visits)
  • Communicate with internal clinician to address any questions or concerns surrounding assessments.
  • Provide immediate correction and education to Home Health Aides if any action is observed during their patient visit that would endanger the patient.
  • Carryout evening and weekend supervisory visits.
  • Review home binder to ensure Home Health Aides are logging consumer's vital signs, weights, blood sugar readings.. etc as indicated.
  • Attend meetings with internal staff, as needed.
  • Performs other duties as assigned by OSH. Follows Open Systems Health care's Policies and Procedures.

Required Credentials:

  • We require an RN license. Have your BSN? Even better!
  • Preferably 1 year of supervisory experience as an RN under your belt.
  • Homecare experience if preferred, but not required.
  • Evidence that youโ€™ve had a Tuberculosis screening that meets CDC guidelines
  • A current and valid CPR certification
  • A reliable form of transportation
