Job description

Schedule/Status: Per diem
Department: Hospice/Home Health
Service Area: Lewiston, Maine
Role: Medical Social Worker - LCSW
Work Setting: Community Care Facilities and Homes

What You’ll Do:

  • Assesses and identify the client’s holistic health care needs/concerns as part of a family and community. 
  • Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
  • Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
  • Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
  • Serve as resource for staff education & training, participate in case conferences as needed.
  • Per Diem LCSW to support Palliative Care, Hospice, and Home Health

What You’ll Bring:

  • Master’s degree from a school of social work accredited by the council on social work education. 
  • Maine Board of Social Work Licensure- LCSW required
  • Current CPR card required. 
  • One or more years of experience as a master’s level social worker in a health care setting.
  • A valid Drivers License and reliable car with insurance.
