Job Overview: We are seeking a compassionate and skilled LCSW therapist to join our team as an in-home counselor to provide mental health support around issues such as mood disorders, grief, and adjustment to illness.

Responsibilities:

Provide Psychotherapy: Deliver individual and family therapy sessions in the person's home setting, which may also be in assisted living or independent living communities. 

Requirements

  Job Overview: We are seeking a compassionate and skilled LCSW therapist to join our team as an in-home counselor to provide mental health support around issues such as mood disorders, grief, and adjustment to illness.

Responsibilities:

Provide Psychotherapy: Deliver individual and family therapy sessions in the person's home setting, including assisted living and independent living communities. 

Client Management: Manage a caseload of 15-25 clients per week, depending on part-time or full-time status.

Assessment and Treatment Planning: Conduct thorough assessments, develop treatment plans, and monitor progress.

Collaboration: Work collaboratively with our home-care teams, including primary care physicians, case managers, and family members.

Documentation: Maintain accurate and up-to-date client records in compliance with HIPAA and other regulatory requirements.

Flexible Scheduling: Participate in scheduled community therapy sessions.

Requirements:

Education: Master's degree in social work (MSW) from an accredited institution.

Licensure: Licensed Clinical Social Worker (LCSW) in the state of Illinois.

Experience: Previous experience in providing psychotherapy or social work services in the home setting preferred.

Skills: Strong assessment, clinical, and communication skills.

Technology: Proficiency and ease with electronic health record (EHR) systems.

Travel: Willingness to travel to the region's home settings as requested.

Compassion: A genuine desire to help individuals improve their mental health and well-being.
