Job description

Bayhealth, Inc. is seeking a Per Diem Home Health RN. The Home Health RN plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.


Responsibilities:

  • Carries out duties as a RN case manager, managing a caseload of homebound patients who require skilled care at home with a multidisciplinary team and the treating physicians.

  • Completes an initial assessment of patient and family to determine the multidisciplinary skilled care needs at home.

  • Develop plan of care based and the comprehensive assessment and review of the History and Physicals (H&P) of the current and previous illness(es), as well as research of the resources available in the agency and the community

  • Develop individualized care plans collaboratively with the patient, family/caregiver in a multidisciplinary approach.

  • Regularly re-evaluates patient nursing needs and revise POC accordingly.

  • Initiates the plan of care and makes necessary revisions as patient status and needs change.

  • Initiates appropriate preventive and rehabilitative nursing procedures.

  • Performs therapeutic procedures and instructs the patient/family/caregiver on the administration of medications and treatments as prescribed by the physician, including, and not limited to oral medications, IV therapy, TPN administration, wound care, enteral feedings and feeding tube maintenance/care; urinary catheter care, tracheostomy care, negative pressure wound therapy, etc.

  • Obtains lab sample as ordered by the treating physician

  • Counsels the patient and family in meeting nursing and related needs.

  • Provides health care instructions to the patient as appropriate per assessment and plan.

  • Establishes discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

  • Provides supervision to the licensed vocational nurse (LVNs) and home health aides (HHAs) to ensure the needs of the patients are being properly met.

Qualifications:

  • Associated Degree in Nursing or higher from an accredited school of nursing

  • Minimum one year of recent acute care experience in an acute care hospital or a skilled nursing facility.

  • One year of home health care experience as an RN

  • Current licensure in State, CPR certification and valid driver’s license.

  • Knowledgeable in OASIS data set, care coordination, and willing to work with a multidisciplinary team.

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

  • Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.

  • Capability of being trained for managing EMR and electronic communication platform for patient care coordination

  • Being on call one weekend per calendar month
