Per Diem RN Case Manager Homecare VNA of Care New England Schools Out. Flexible Nursing Opportunities Are In.

Are you a school nurse or RN looking for a flexible summer opportunity that allows you to continue making a meaningful impact in patient care?

The VNA of Care New England is seeking compassionate and motivated Per Diem RN Case Managers to join our Homecare team. This role offers the flexibility to work around your summer schedule while providing one-on-one patient-centered care within the community.

As an RN Home Health Clinician, youll manage a caseload of patients from admission through discharge while collaborating with a dedicated interdisciplinary team to support positive patient outcomes.


What Youll Do
  • Perform comprehensive patient assessments and develop individualized care plans
  • Coordinate care and referrals across disciplines and specialties
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and caregivers on managing care at home
  • Communicate effectively with patients, families, and healthcare providers

Qualifications
  • Current Rhode Island Registered Nurse (RN) license required
