What is a peer bridger?

Peer Bridgers are individuals with lived experience of mental health or substance use challenges who provide support to others transitioning from inpatient or institutional care back into the community. They use their own recovery journey to offer guidance, encouragement, and practical assistance, helping peers navigate services and build connections. Peer Bridgers play a crucial role in reducing isolation, promoting recovery, and improving outcomes for individuals re-entering the community.

What does a peer bridger do?

As a Peer Bridger, you play a vital role in helping individuals move from inpatient mental health settings back to their communities. Your work often involves providing emotional support, sharing personal recovery experiences, and assisting clients in connecting with local resources such as housing, employment, or outpatient services. You'll collaborate closely with clinical teams, case managers, and community partners to ensure a smooth transition and continuity of care. This supportive, relationship-based approach helps reduce hospital readmissions and empowers clients to achieve greater independence.
