Job description

A Nursing home is seeking a detail-oriented and experienced MDS Coordinator to join their clinical team. This role is ideal for a nurse who is strong in assessment, documentation, and care planning—and who understands how to drive accurate reimbursement and quality outcomes.

Key Responsibilities
  • Complete and manage the full RAI/MDS process in accordance with federal and state guidelines.

  • Conduct resident assessments and develop individualized care plans.

  • Collaborate with nursing, therapy, social services, and dietary teams.

  • Ensure compliance with PDPM, CMS regulations, and facility policies.

  • Monitor deadlines, coding accuracy, and documentation requirements.
