Job description

Cobble Hill Health Center is seeking an experienced RN Director of MDS to lead and oversee all aspects of the MDS process in our well-established, not-for-profit long-term care facility.

This is a key leadership role responsible for ensuring accurate assessments, regulatory compliance, and optimal reimbursement outcomes.

Responsibilities
  • Oversee all MDS assessments and submissions
  • Monitor and improve Case Mix Index (CMI)
  • Perform PDPM reviews on admission and prior to submission
  • Ensure accurate MDS coding and documentation
  • Collaborate with rehab and interdisciplinary teams
  • Review UB04s and coordinate with billing
  • Manage ADR responses, audits, and appeals
  • Track and present Quality Measures
  • Run Medicare Part B reports
  • Participate in QAPI, Utilization Review, Therapy, and Medical Board meetings
Requirements
  • Active RN License โ€“ New York
  • 3+ years experience as MDS Coordinator or MDS Director
  • Strong knowledge of PDPM, Medicare, and Medicaid
  • Experience with CMI optimization and compliance
  • Excellent communication and leadership skills
Preferred:
  • RAC-CT Certification
  • Prior leadership experience
