Job description

PCC Community Wellness Center is seeking compassionate, mission-driven Registered Nurses to support patient care across our Chicago and near-west suburban health centers. This role provides clinical and administrative support to providers and care teams while ensuring an exceptional patient experience. Nurses will work across various PCC locations, including Austin, ARC, Steinmetz, Salud, West Town/Boulevard, Lake Street, Melrose Park, Berwyn (PCC South), and West Suburban Hospital sites, based on staffing needs.


Shifts may range between 8:00 AM and 8:00 PM depending on site needs, with occasional Saturday coverage required.

Essential Duties & Responsibilities:

Reports to: Director of Team Nursing

FLSA Status: Non-Exempt

Position Summary: The Registered Nurse delivers high-quality patient care by triaging calls and walk-in patients, supporting smooth patient flow, and assisting providers with exams, treatments, and procedures. This role performs and documents patient assessments, administers medications, conducts basic lab tests, provides patient education, and ensures accurate EMR documentation. The Nurse also supports quality improvement efforts, maintains a safe and organized clinical environment, and collaborates with the care team to ensure excellent patient service.

  • Triages emergency and non-emergency phone calls and walk-inpatients providing appropriate direction. Documents according to PCC’s clinic policy and procedures.
  • Directs patient flow.
  • Assists other staff in need of assistance to maintain patient flow.
  • Assists providers with treatments and procedures as directed.
  • Assesses the patient’s condition and learning needs in regard to health care.
  • Utilizes teach back method to provide necessary patient education.
  • Performs and documents basic patient history, treatments, and administered medications appropriately.
  • Administers prescribed medications and treatments as directed by the provider following PCC policies and procedures.
  • May obtain vital signs and perform routine functions to prepare patients for examinations or procedures.
  • Maintains accurate EMR and reports to the provider any pertinent information regarding patient’s condition and care.
  • May perform laboratory and diagnostic tests as necessary.
  • Reviews patient’s records and other pertinent information, takes appropriate action.
  • Keeps providers informed of changes in assignments or patient flow that will affect working conditions.
  • Performs laboratory and diagnostic tests as ordered by the provider.
  • Ensures work area is stocked with necessary supplies and materials.
  • Performs and records waived testing competency.
  • Assesses emergency equipment in operational mode at all times; reports any equipment not functioning.
  • Cooperates with other health care personnel to coordinate patient care and establish mutual goals.
  • Notifies patient by telephone or certified mail of any abnormal test results as directed by provider.
  • Assists clinical staff with the orientation of new staff.
  • Assesses educational needs of clinical staff, provides additional training as needed.
  • Initiates and pursues own plan for continued professional growth.
  • Maintains patient confidentiality.
  • Complies with PCC policies, procedures, and safety practices.
  • Functions as a resource for all clinical and non-clinical support staff.
  • Maintains a neat and orderly clinic work area.
  • Complies with PCC customer service guidelines and protocols.
  • Drive continual quality improvement through participation in organizational quality improvement efforts that may include but are not limited to patient satisfaction, clinical and operational quality improvement initiatives.
  • Performs other duties as assigned.

Experience/Training:

  • Graduate of an accredited school of nursing.
  • Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.
  • 2 – 3 years clinical experience preferred.
  • 2 - 3 years EMR experience preferred.
  • Proficient in MS Office Business Applications to include: Outlook, Word, PowerPoint, and Excel.
  • Certifications/Licenses:
  • Current and valid State of Illinois nurse’s license.
  • Current and valid CPR card.
  • Physical Demands:
  • Must be able to remain in a stationary position 50% of the time.
  • Must be able to move around the clinic site 50% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  • Ability to move as needed to perform necessary nursing skills such as wound care, injections, and other procedures.
  • Must be able to transport from one site to another.
  • Must be able to cover other shifts as necessary.
