Position: Patient Care Coordinator
Location: Layton
Type: Part-Time
Schedule: Saturday and Sunday 3pm - 9pm
Job Description:
As a Care Coordinator at Wee Care Pediatrics, you play a crucial role in ensuring the seamless and coordinated care of our young patients. Your responsibilities include collaborating with medical professionals, facilitating referrals, managing prescription fulfillment, overseeing lab results, serving as a patient point of contact, and ensuring the continuity and quality of care.
Job Duties:
  • Provide patients with appropriate referrals to specialists and additional healthcare services.
  • Manage prescription fulfillment and refills, ensuring timely access and accurate records.
  • Receive, document, and communicate lab results for timely follow-up and care decisions.
  • Act as a patient advocate, addressing questions and providing support and information.
  • Coordinate patient care transfers between departments or facilities for continuity of care.

Job Qualifications
  • Previous MA work experience desired; experience in Pediatrics preferred.
  • Understand various lab tests and able to clearly communicate results and provider instructions to parents.
  • Answer basic medical questions and offer advice to parents over the phone ensuring high standard of care.
  • Ability to communicate effectively with providers, staff, and patients/parents.
  • Capable of prioritizing multiple job demands and work in a fast-paced health care environment.
  • Demonstrate compassion and understanding for the patient and families.
  • Basic computer knowledge.
