As a Casual Patient Navigator, you are organized, adaptable, and committed to supporting the health and wellbeing of community members. You are comfortable coordinating services, advocating for patients, and working collaboratively with healthcare providers, families, and community partners to ensure members receive the care and resources they need.
ROLES AND RESPONSIBILITIES:
- Provide compassionate, patient-centered support to members and their families throughout their healthcare journey.
- Coordinate transportation arrangements for medical appointments in collaboration with the Medical Transportation Program, ensuring timely and efficient travel arrangements.
- Assist patients with scheduling medical appointments and coordinating healthcare-related services.
- Provide wellness visits, hospital visits, palliative support, and emotional support to patients and families as required.
- Facilitate communication between patients, families, healthcare providers, and support services.
- Connect patients and families with available community resources, including Non-Insured Health Benefits (NIHB) and other support programs.
- Maintain accurate, confidential, and up-to-date records of patient interactions, referrals, and services provided.
- Support health promotion initiatives, wellness programs, community engagement activities, and cultural events.
- Provide information, guidance, and advocacy to assist patients in navigating healthcare systems and services.
- Work collaboratively with healthcare professionals, community partners, and internal departments to ensure continuity of care.
- Maintain confidentiality and adhere to organizational policies, procedures, and professional standards.
- Perform other related duties as assigned.
EDUCATION AND EXPERIENCE:
- High school diploma or equivalent is required.
- Post-secondary education in Health Studies, Social Work, Human Services, Community Health, or a related field is preferred.
- Additional healthcare support, patient navigation, social services, or related training is considered an asset.
- Experience working with First Nations communities, organizations, and partners is required.
- Experience providing support to individuals and families in healthcare, wellness, social services, or community-based settings is considered an asset.
- Strong verbal and written communication skills.
- Demonstrated ability to build positive relationships with community members, healthcare providers, and stakeholders.
- Strong organizational, time management, and problem-solving skills.
- Proficiency in Microsoft Office applications.
- Knowledge of Indigenous culture, traditions, and community resources is considered an asset.
OTHER REQUIREMENTS:
- Valid Class 5 Alberta Driver's License.
- Clean Driver's Abstract.
- Access to a reliable vehicle for work-related travel.
- Willingness and ability to travel as required.
- Willingness and ability to work flexible hours, including evenings, weekends, and on-call shifts when required.
- Successful completion of a Criminal Record Check.
- Willingness to sign and adhere to an Oath of Confidentiality.
- Ability to work independently and as part of a team.
- Strong working knowledge of Microsoft Office Suite.
- Excellent written and verbal communication skills in English.
- Ability to provide clear, accurate, and professional information to clients and stakeholders.
- Ability to speak and/or understand Cree or another Indigenous language is considered an asset.
WORKING CONDITIONS:
- Frequent travel to hospitals, healthcare facilities, community locations, patients' homes, and wellness-related events.
- Participation in community engagement activities, cultural events, health promotion initiatives, and wellness programming.
- Ability to work in healthcare, office, community, and home environments.
- Occasional lifting and carrying of lightweight supplies, equipment, or materials.
- Exposure to varying environmental conditions, including healthcare settings and community locations.
- Flexibility to work extended hours, evenings, weekends, and respond to urgent situations when required.
- Ability to manage the emotional demands associated with supporting patients and families experiencing illness, grief, loss, or other health-related challenges.
- Compliance with infection prevention and control protocols, including the use of Personal Protective Equipment (PPE) when required.
- Ability to maintain professionalism, compassion, and resilience while working in sensitive and emotionally demanding situations.
MCFN Standard Closing Statement
In accordance with MCFN hiring practices, this position will be posted internally for a minimum of fourteen (14) days to provide qualified MCFN Members and internal candidates with the opportunity to apply.
