Human Touch Home Health is looking for an experienced Intake Coordinator to join our Falls Church, Virginia office. This is a full-time, in-office position for someone who understands the pace of home health and knows how to keep referrals moving smoothly from start to finish.

What You'll Do
  • Receive and process new referrals from hospitals, physicians, and case managers
  • Verify insurance and obtain authorizations as needed
  • Enter patient information accurately into the EMR
  • Coordinate with clinical managers and field staff to ensure timely admissions
  • Communicate with referral sources, patients, and internal teams
  • Track referral status and follow up on missing documentation
