Job description

Living Room Crisis Specialist/Case Manager - Part-Time

Under the assigned supervisor, the Living Room Program Crisis Specialist/Case Manager provides on-site crisis intervention, case management, counseling, or mental health services to adults in accordance with the agency's policies and program guidelines. The Crisis Specialist/Case Manager works to alleviate or stabilize mental health and behavioral health symptoms of referred clients that impair their ability to adjust to home, integrate into the community, or maintain living stability. Services are provided to individuals in crisis on a 24/7/365 basis inside the Living Room location.
Essential Duties and Responsibilities
  • Provides Crisis Intervention Services within scheduled shift. Program operates 24/7/365, so schedules include evenings, overnights, weekends, and holidays.
  • Some additional coverage of nights, weekends, and holidays if schedule or staffing requires additional support.
  • Works part-time hours (usually 20-35 hours per week); however, this is flexible based on program needs.
  • Conducts In-Person Crisis/Safety Assessments with Peer Support professional
  • Works with the Peer Support professional to use assessment tools to determine the immediate level of care needs and the service array for referred clients.
  • Facilitates linkage to necessary level of care for the individual in crisis, including but not limited to: psychiatric hospitals, shelter resources, food banks, insurance and medical resources, employment resources.
  • Provides follow-up services after a crisis, including but not limited to linkages to resources, Mental Health Assessments, Treatment Plans, and other Case Management services in accordance with the agency’s trauma-informed Framework (ARC).
  • Provides mental health stabilization and specific services according to funding source (such as Medicaid) where applicable. 
  • Completes required paperwork within established timelines and Medicaid-compliant documentation, including but not limited to: assessment documentation, case notes, creation and maintenance of physical client file
  • Provides transportation for clients in accordance with program guidelines. 
  • Assists in the integration of all services provided for the client by serving as a central point of communications for service components during crisis 
  • Represents the agency at court hearings as necessary 
  • Performs other related duties as assigned.

Qualifications
  • Education: BS/BA degree in a human service-related field
  • Experience: 1- 3 or more years related experience, preferred
  • Other: Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance

  • Highly organized, yet flexible, and can handle multiple priorities/projects well; ability to manage tight deadlines.
