Leveraging Equal Access Program (LEAP), is a 501 (c)(3) nonprofit organization that helps individuals with intellectual and developmental disabilities. Our individuals' disabilities range from Down syndrome, MR, Traumatic Brain Injury, and Autism, in addition to other cognitive and physical disabilities. Our team is creative, extraordinary, dynamic, and committed to making a positive impact on the developmentally disabled community.

We are hiring a part-time Social Worker.

Location- We are seeking applicants only located in the San Gabriel/Pomona area
Part Time (approx. 20 hrs per week)
Hybrid with Travel
Reports to: LEAP Director

As a Social Worker at LEAP, you'll play a vital role in supporting individuals with disabilities and their home providers. This is a part-time position at our San Gabriel/Pomona location, designed to provide clinical case management and ensure continuity of care. We're seeking a compassionate, skilled professional who thrives in a collaborative and flexible environment. Clinical supervision for ASWs is available.

RESPONSIBILITIES

  • Carry a small caseload, providing assessment, service planning, coordination, and follow-up.
  • Conduct home and community visits as needed to monitor individual well-being and service effectiveness.
  • Offer clinical support including psychoeducation, advocacy, mediation, and crisis intervention.
  • Ensure all documentation and contact logs meet legal, regulatory, and internal policy standards.
  • Perform comprehensive needs assessments and connect individuals to appropriate services such as healthcare, outpatient programs, recreational resources, and other community supports.
  • Collaborate with internal departments and external providers, including LEAP Consultants, to coordinate care and ensure continuity of services.
  • Participate in team meetings, trainings, and supervision when applicable.
  • Maintain compliance with ethical standards, confidentiality laws, and professional boundaries at all times.

SKILLS & COMPETENCIES

  • Highly skilled in de-escalation techniques, crisis response, and assessing safety risks for individuals with complex behavioral health needs.
  • Strong interpersonal skills to engage effectively with individuals, families, and interdisciplinary teams, fostering trust and teamwork across systems of care.
  • Maintains confidentiality, exercises sound judgment, and upholds ethical standards in all interactions, including the handling of sensitive and protected information.
  • In-depth knowledge of community-based services and public systems, including SSI, Medi-Cal, APS, CPS, DDS, Regional Centers, Medical Centers, transportation agencies, and the legal/forensic programs.
  • Well-versed in behavioral health laws and ethical frameworks, including mandated reporting requirements and the NASW Code of Ethics.

      WORK ENVIRONMENT & SCHEDULE

      • Setting: Hybrid role; combination of remote work and in-person visits within the El Monte, Foothill, and Pomona areas of Los Angeles County.
      • Travel: Requires reliable transportation and the ability to travel locally for home and community visits.

        EDUCATION & EXPERIENCE

        • Master's Degree in Social Work (MSW) from an accredited university is required.
        • LCSW or ASW (registered with the BBS) required.
        • Minimum 3 years' experience in case management, behavioral health, or developmental disability services.
        • Experience working with Regional Centers, supported living providers, or similar systems strongly preferred.
        • Experience providing therapy, crisis intervention, or clinical case management is highly valued.
