Job Responsibilities:

  • Conduct skilled nursing visits for patients in their homes.
  • Engage in direct patient interactions to assess health conditions.
  • Educate patients and families about health management, including CHF, COPD, diabetes, etc.
  • Provide wound care and medication management.
  • Document patient conditions and report significant changes to the physician.
  • Collaborate with a team of other healthcare professionals, including Physical Therapists and Occupational Therapists.

Requirements

  • Current RN Nursing License in the state of practice.
