• Conduct thorough psychosocial assessments to understand each client's unique background, mental health needs, and available resources.
• Provide individual, family, and group therapy, employing evidence-based therapeutic modalities tailored to the hospice population.
• Develop comprehensive treatment plans that address both the psychological and emotional challenges presented by serious and terminal illness.
• Work collaboratively with an interdisciplinary team, including physicians, nurses, and chaplains, to develop a coordinated care approach that best supports our clients and their families.
• Facilitate therapeutic support groups and psychoeducation sessions that empower clients and family members with knowledge and coping strategies.
• Engage in crisis intervention and safety planning to ensure the well-being of patients facing acute emotional distress.
• Maintain meticulous and timely documentation in compliance with all ethical and legal standards, ensuring that client records are comprehensive and up to date.

The core of our work focuses on patients diagnosed with terminal illnesses, their families, and caregivers. In this role, you will support clients dealing with the emotional and psychological aspects of end-of-life care. You will be instrumental in helping clients navigate feelings of grief, fear, and uncertainty, fostering resilience and encouraging healing during challenging times.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Licensed Certified Social Worker (LCSW) in the state of Kansas.
• Strong foundation in evidence-based therapeutic practices and modalities applicable to hospice and palliative care.

• A minimum of two years of clinical experience in mental health or outpatient social work settings, preferably within hospice or palliative care.
• Demonstrated expertise in conducting psychosocial assessments and developing patient-centered treatment plans.
• Familiarity with telehealth systems and digital communication tools to provide remote care effectively.
