Alvita Counseling Center is seeking a dedicated and skilled Licensed Clinical Social Worker (LCSW) to join our remote team specializing in hospice care. Although primarily remote, occasional meetings via telehealth platforms will facilitate collaboration and professional dialogue to ensure seamless patient care. • Conduct thorough psychosocial assessments through interviews and observations, factoring in emotional, social, and environmental influences that may affect patient recovery and well-being.
• Deliver empathetic counseling and crisis intervention to patients and families coping with life-limiting illnesses, facilitating discussions around adjustments and emotional wellness.
• Collaborate with the rehabilitation and hospice teams to formulate tailored treatment plans that incorporate both medical and psychosocial needs.
• Assist patients and families in navigating community resources, such as support networks and bereavement services, that can enhance their quality of life.
• Guide families through post-hospice support services and plans, ensuring a smooth transition back into the community.
• Engage in regular discharge planning to prepare patients for changes in care settings, whether returning home or transferring to another care facility.

The ideal LCSW will work with a diverse patient population facing complex end-of-life challenges, providing social work intervention that addresses both acute and chronic stressors. Your role will involve understanding and addressing the unique psychosocial dynamics involved with progressing towards end-of-life care, including facilitating advanced care planning discussions and grief responses in families.

• A Master's degree in Social Work from an accredited institution is required.
• Validation through active state licensure as a Licensed Clinical Social Worker (LCSW).
• A deep understanding of grief, adjustment disorders, and mechanisms for coping with chronic illnesses and end-of-life issues are essential.
• Exemplary communication and interpersonal skills, with the capacity to build rapport with patients and their families.

• Previous experience within a hospice, rehabilitation, or palliative care setting is strongly preferred.
• Experience in conducting psychosocial assessments and crisis intervention in challenging patient demographics is an asset.
• Familiarity with community resources relevant to hospice and palliative care will enhance service delivery.
