This position is performed remotely, providing you with the flexibility to work from home while ensuring that you are connected with our interdisciplinary team. Key Responsibilities
  • Conduct thorough client assessments to identify mental health needs and develop individualized treatment plans that reflect patients’ preferences and goals.
  • Facilitate therapeutic sessions for individuals, couples, and families, utilizing evidence-based techniques to promote healing and resilience during challenging times.
  • Collaborate closely with a multidisciplinary team, including physicians and nursing staff, to ensure holistic and coordinated care for patients within the hospice setting.
  • Document patient interactions meticulously in accordance with legal and ethical standards, maintaining accurate records and progress notes for ongoing treatment evaluations.
  • Participate in regular supervision and professional development sessions to enhance clinical skills and stay current with best practices in social work and hospice care.

Patient Population / Client Focus

You will work primarily with terminally ill patients and their families, providing emotional support, counseling, and advocacy throughout the hospice journey. Your expertise will be pivotal in helping patients navigate complex emotions associated with end-of-life issues, facilitating family discussions, and providing grief counseling to bereaved loved ones. This sensitive and impactful work requires a deep understanding of both mental health and the specific challenges faced in hospice care.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • Current licensure as a Clinical Social Worker (LCSW) in the state of Kansas.
  • Minimum of three years of relevant experience in clinical social work, preferably in a healthcare or hospice setting.
  • Strong foundational knowledge of mental health diagnoses, treatment modalities, and crisis intervention techniques.
  • Exceptional communication skills, both oral and written, and the ability to connect with patients and families compassionately.

Preferred Experience
