Job description

Job Description
Completion of release of information requests in accordance with HIPAA/confidentiality regulations and established departmental turnaround time. Accurate/timely completion of Birth Certificates/IPHIS abstraction in accordance with state mandates. Ensuring the data integrity of patient medical records across hospital wide patient information systems.
Responsibilities
Demonstrated competence with clinical applications for health record processing. Proficiently navigates through the clinical applications to identify patient demographics, provide patient information to appropriate requestors in timely manner and verifies the accuracy of the demographic information.
Responsible for the confidentiality and security of all patient health records for the hospital. Understands all rules and regulations regarding the proper release of patient information. Keep up to date on any changes with rules and regulations associated with patient confidentiality and privacy.
Ability to complete Birth Certificates for patients in a timely/accurate manner and in accordance with state timelines-inclusive of data collection, affidavit completion, and IPHIS abstracting.
Monitors the data integrity of patient's medical record across all patient information systems. Proficiently navigates through software applications utilized in MPI validation, inclusive of duplicate medical record numbers, patient merge/unmerges overlays and name discrepancies.
Good customer services skills and excellent communication skills for handling all release of information requests received from our patients, insurance companies or other entities.
Ability to meet productivity and quality standards according to department policy and procedure for each job function. (Meets standards - does well; Below standards - needs improvement.
Accurate completion of all other duties as assigned by the HIM Mgt team.
Qualifications
Minimum Required: High school graduate required. Preferred: Associate Degree. | Registered Health Information Technician (RHIT) preferred, (or RHIT eligible). | 2 years experience in Health Information Management or related health care/office experience.
