Description
As a QMAP/Caregiver, your primary responsibility is to provide personal care and to administer medications, as prescribed and within the scope outlined in the relevant regulations, to the residents in our community. If you have not received your QMAP, please only perform caregiving duties. Once you have received your QMAP and have gone through QMAP training and had a satisfactory shadowing review by a QMAP Trainer/Coordinator, you will perform the QMAP duties as outlined in this job description. Delegated responsibility to perform duties as stated in The Community Policies and Procedures and in compliance with all relevant rules and regulations.
Caregiving duties:
Answers call lights, assists with daily tasks, assists with teeth/dentures, grooming including shampooing, combing hair and shaving, assists with bathing, dressing and undressing residents. Assists with personal laundry, making beds and taking out trash.
• Documents and charts promptly and accurately. Monitors and charts resident's activities when they're out of the building and when/if they are eating. Assists residents with Activities of Daily Living (ADL) and charts accurately. Uses Yardi platform regularly and ensures professionalism while charting (reporting facts, not opinions)
• Communicates professionally and effectively with residents, families and co-workers. Regularly uses a functional two-way radio to communicate. Asks for assistance and offers assistance to co-workers
• Escorts residents to and from meals, activities, and assists them using their walkers and wheelchairs
• Takes vital signs or observes possible adverse responses to medication and reports to the nurse-on-duty or Wellness Director
• Providing incontinent care properly and consistently and assists with transferring residents
• Communicates resident changes to the in-coming shift, not leaving shift until information has been exchanged and ensures details are in progress notes on Yardi
• Light housekeeping such as making beds, emptying trash and clean up as needed to ensure residents' apartments are in the best condition
• Other duties as assigned by the Executive Director, Wellness Director or Care Coordinator
• Present a professional demeanor that communicates the Community philosophy to all residents and staff
• Report all emergency situations, reportable incidents, staffing issues and observable changes in resident behaviors and/or health status to their direct supervisor
QMAP duties
• Administers medications to residents based on the seven Rights; Person, Right Drug, Right Dose, Right Route, Right Time, Right Documentation, Right of the Client to Refuse
• Reorders medication in a timely manner
• Follows Community Policies and Procedures on medication administration
• Keeps medication cart clean and well stocked, reporting any out-of-stock medicine and refused medicine to the appropriate Community resource
• Charts all medication administrations in a timely and accurate manner on Yardi
• Adheres to the QMAP Scope and Standards for practice in Colorado
Qualifications and Experience
• Minimum High School Diploma or GED preferred
• Eager to Learn and to be a Part of a Strong Team
• Pass Background Check and Drug Tests
• Pass Routine TB Test
• Able to use general office equipment and computer systems required.
• Must demonstrate dependable, regular attendance
• Must be able to communicate and function productively on an interdisciplinary team
• Has familiarity and willingness to work with elderly population
• Receives and maintains CPR/First Aid certification
• QMAP must complete Colorado QMAP class, pass written and practical test, and has never had a professional license or certificate revoked by a state or any other jurisdiction.
Must have a record of satisfactory med pours

Working conditions:
May be exposed to infectious waste, diseases, conditions, etc. Please wear Personal Protective Equipment (PPE) provided.
Physical requirements:
Must be able to meet physical demands of lifting and moving over 30 pounds, sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours. Moving at a high pace for long periods of time.
