Job Description: EdAdvance is seeking a Part Time-Per Diem Physical Therapist for the Birth to Three Department. The qualified candidate will be professional, organized, and able to work independently.
Major Responsibilities:
  • Home visitation with infants, toddlers, and their families
  • Assist families with the implementation of strategies to enhance the development of their child
  • Develop IFSP (Individualized Family Service Plan) with families
  • Provide service coordination to families
  • Assist child with the transition to preschool programming
  • Work in trans-disciplinary team of Early Childhood professionals
  • Other duties as may be assigned
  • Regular and consistent attendance is considered an essential function of this position

Knowledge, Skills, and Abilities:
  • Working knowledge of Microsoft Office including Excel
  • Ability to develop effective relationships
  • Excellent interpersonal skills

Qualifications/Requirements:
  • CT DPH Licensed/Certified Physical Therapist
  • Experience with infants, toddlers, and families
  • Service Coordination training (or willingness to obtain)
  • Excellent organizational skills and computer literacy

Physical Requirements:
  • Frequent standing, bending, kneeling and therapeutic holding as required
  • Must be able to perform essential functions with or without accommodations

Travel: Travel required within the Torrington area. Must have reliable transportation, have a driver's license, and car insurance.
