Position Overview:
We are seeking an experienced, self-driven Full-Time or Part-Time (Per Diem) Physical Therapist (PT) for Health Care to join our dedicated team and provide exceptional care throughout the region. As a PT, you will evaluate patients' physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities:
โ€ข Provide physical therapy services within the scope of care, following the patient's individualized plan, in coordination with the Interdisciplinary Team.
โ€ข Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
โ€ข Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
โ€ข Comply with all company policies and procedures to ensure high standards of care.
Requirements:
โ€ข Current Physical Therapist (PT) License to practice in California - Required
โ€ข Experience in Home Health or Hospice care - 1 Year Required
โ€ข Current CA Driver's License.
โ€ข Able to travel within a 15 - 25 mile radius on a regular basis.
