Position Summary:
HiHealthCare is currently seeking experienced and compassionate Licensed Practical Nurses (LPNs) to provide skilled nursing care to clients in their homes. This role offers the flexibility of Part Time and Per Diem schedules, making it ideal for LPNs looking to balance work with other personal or professional commitments.  
Responsibilities: 
  • Provide direct nursing care under the supervision of a Registered Nurse and in accordance with a patient’s individualized care plan
  • Conduct home visits to assist in monitoring patient health and home environment
  • Administer medications and treatments as prescribed
  • Monitor and document changes in patient condition
  • Communicate effectively with patients, families, supervising RNs, and interdisciplinary team members
  • Assist in educating patients and caregivers on basic health maintenance
  • Ensure compliance with all clinical and safety protocols
Qualifications:
 
  • Current and unrestricted LPN license in the State of Hawaii
  • Minimum of one (1) year nursing experience; home care experience preferred
  • CPR certification (BLS)
  • Strong clinical skills and attention to detail
  • Excellent communication and interpersonal skills
  • Valid driver’s license, reliable transportation, and proof of auto insurance
