Role Overview:
The LPN will provide in-home nursing care to patients under the supervision of a Registered Nurse. You will be part of an interdisciplinary team dedicated to ensuring comfort, quality of life, and dignity for every patient and their family.
Your Key Responsibilities Will Include:

  • Administering medications and treatments following hospice protocols and nursing standards
  • Performing essential clinical tasks within the scope of LPN practice
  • Monitoring changes in patient condition and notifying the RN as needed
  • Contributing to the development and revision of care plans with measurable outcomes
  • Maintaining complete and timely documentation of all care activities
