Description
Job specific duties:
-Assess and treat patient
-Facilitate group treatment
-Conduct assessment of new family and develop initial treatment plan for family
-Develop master treatment plans in a timely manner as prescribed by program policies and procedures using data from all assessments and input from treatment team reviews and updates treatment plans for assigned patients on a timely basis as prescribed in the program policies and procedures
-Effectively utilize interdisciplinary planning meetings to review proper problem problems, progress and discharge plan for all assigned patients
-Enter progress notes within the established timeframe for all assigned patients; progress notes are accurate, refers to problems identified on treatment plan, addresses progress and gains as related to treatment goals
-Document incidents or unusual occurrences at the time it happens
-Monitor medical records for all assigned patients to ensure patient safety and overall care coordination
-Complete discharge summaries for all assigned patients in a timely manner as prescribed by program policies and procedures
-Experience and confidence identifying and implementing evidence based practices through trauma informed care lens
-Develops therapeutic relationships with assigned patients and their families; schedule Weekly 1:1 and family sessions based on pt unique needs.
-Effectively develop and review weekly treatment plans, goals and progress with assigned patients
-For each assigned patient, act as liaison/advocate for information regarding medication changes, treatment, changes in treatment plan, discharge planning, etc.
-Regularly monitor patient attendance, motivationally interviewing pt and practicing coping skills with pt and their family
-Effectively gather information to the pt and family benefit
-Effectively assist patients in advocating for their needs
-Effectively coordinate with nurse, attending physician or PA-C regarding treatment plan, progress and discharge plans
-Follows mandatory reporting laws for children , disabled or older adults and other protected populations in a timely manner as indicated by California Regulations
-Regularly follows all individual and group billing procedures and accurately enters information on correct form
-Manage patient crisis situations
Requirements
- Doctorate in Clinical Psychology
- In good standing with the Board of Behavioral Services
- 7 year clinical experience with adults, adolescents, mentally ill and/or developmentally disabled
- 5 year experience with the substance use disorder population
- Effective written/verbal communication in English
- Understanding of psychiatric diagnosis, terminology, and medical record charting basics
- Experience in group facilitation and documentation
