Referral Marketing Representative – HealthcareExecutive Home Care of Omaha – Omaha, NE

About the Role
Executive Home Care of Omaha is looking for a Referral Marketing Representative to build and strengthen relationships with Skilled Nursing Facilities (SNFs), rehabilitation centers, Assisted Living Facilities (ALFs), and Memory Care communities throughout the Omaha area.
This is a relationship-building and business development position — not a clinical care role. You will represent Executive Home Care in the community and develop trusted relationships with discharge planners, social workers, case managers, and facility staff who help families transition to in-home care.
This opportunity is especially well suited for someone who has worked as a social worker, discharge planner, case manager, or healthcare professional and understands how referral decisions are made.
You will use your knowledge of the healthcare community to help facility partners feel confident referring their patients and residents to Executive Home Care for safe, dependable, high-quality in-home care.
What You'll Do
  • Build and maintain strong professional relationships with discharge planners, social workers, case managers, and facility staff
  • Develop relationships with SNFs, rehab facilities, ALFs, and Memory Care communities throughout the Omaha metro
  • Conduct regular in-person facility visits, introductions, and follow-ups
  • Present Executive Home Care's services and value proposition to healthcare and facility professionals
  • Educate referral partners about EHC programs, including Sensi Co-Pilot and the GUIDE Program
  • Serve as a reliable point of contact for referral partners and respond to questions or requests
  • Deliver marketing materials and maintain consistent communication with referral sources
  • Identify new referral and partnership opportunities
  • Track outreach activities, follow-ups, referrals, and relationship development
  • Help grow and strengthen EHC's referral network throughout the Omaha area
  • Represent Executive Home Care at community events, lunch-and-learns, and in-service presentations
  • Collaborate directly with EHC leadership on outreach strategy, messaging, and territory development
Qualifications
  • 5+ years of experience in social work, discharge planning, case management, healthcare, or a related field
  • Experience working with hospitals, SNFs, rehab facilities, Assisted Living, Memory Care, or other senior care settings strongly preferred
  • Understanding of the healthcare referral and discharge planning process
  • Familiarity with the Omaha-area healthcare and senior care community is a strong plus
  • Comfortable with face-to-face networking, facility visits, and professional outreach
  • Excellent communication and relationship-building skills
  • Professional, personable, and confident when interacting with healthcare professionals
  • Self-motivated and able to manage your own schedule, territory, and follow-up activities
  • Strong organizational and follow-through skills
  • Reliable transportation and ability to travel throughout the Omaha metro
  • Genuine interest in helping seniors and families access quality in-home care
