Wellness Coach (or case manager)

Program: Mental Health Group Home

Location: New Castle County ( Wilmington and Newark, DE)

Status: PRN or Part-Time - hours vary

Schedule: TBD

ABOUT THE ROLE

The Wellness Coach works under the authority of the Assistant Director to ensure operational procedures are followed. This role uses psychiatric rehabilitation principles to promote recovery, community integration, and improved quality of life for residents. The Wellness Coach collaborates with consumers, team members, families, and other resources while supporting the Psychiatric Rehabilitation Model in daily practice.


RESPONSIBILITIES

  • Utilize psychiatric rehabilitation principles to foster recovery and community integration.
  • Maintain a safe, supportive, and recovery-focused environment that encourages growth and rehabilitation.
  • Engage consumers in trusting, supportive relationships and provide emotional support within professional boundaries.
  • Support individualized recovery plans to help consumers achieve physical, mental, and emotional well-being.
  • Assist consumers in reaching goals outlined in their recovery plans.
  • Encourage and practice advocacy activities to promote self-advocacy.

REQUIREMENTS

  • Bachelor’s Degree in a Human Service discipline.
  • Valid driver’s license.

ADDITIONAL QUALIFICATIONS

  • Ability to lift up to 50 pounds occasionally (medium physical demand).
  • Comfortable working in varied conditions (heat, cold, dust, fumes, confined spaces).
  • Physical abilities include standing, sitting, walking, climbing, reaching, kneeling, and repetitive motion.
  • Strong interpersonal and communication skills.

RESOURCES FOR HUMAN DEVELOPMENT (RHD)

RHD Delaware Residential Programs provide 24/7 residential services to adults with serious mental illness. The four homes accept referrals from Delaware’s Division of Substance Abuse and Mental Health to individuals who are approved for group home level of care. The program provides services utilizing a trauma-informed approach, along with other evidence-based practices. Support is focused on the areas of illness management, building skills of community living, maximizing quality of life, and reducing the need for inpatient hospitalizations. Residents contribute a portion of their monthly income toward room and board.
