Position Overview: As a Palliative/Hospice Nurse Practitioner at Salus Hospice, you will work closely with a physician and interdisciplinary team to provide compassionate, patient-centered care. You'll help ensure hospice patients meet the face-to-face visit requirements and will support the team in delivering exceptional care to patients and their families.
Key Responsibilities:
  • Conduct face-to-face patient visits to evaluate continued hospice eligibility in line with new regulations.
  • Collaborate with physicians and the interdisciplinary team to share clinical findings and adjust care plans.
  • Perform bedside evaluations, creating comprehensive care plans tailored to the patient's needs.
  • Provide direct hospice care, supporting both the patient and family through sensitive and crucial moments.
  • Document all findings per CMS guidelines, ensuring accurate compliance.
  • Consult with supervising physicians and collaborate with healthcare providers within and outside of Salus Hospice.

What We're Looking For:
  • 2+ years of recent experience in hospice, or palliative care
  • Expertise in providing care to patients with end-stage disease.
  • Master's degree in Nursing preferred.
  • Active Nurse Practitioner certification in California.
  • California state licensure to practice as a Nurse Practitioner.
  • Willingness to travel within a large geographic area to visit patients.
