Part-Time Occupational Therapist - Home Health | Riverside County, CA (3 Days/Week)

What You'll Do

  • Conduct initial and ongoing occupational therapy evaluations in the home
  • Assess patient functional status, including strength, endurance, coordination, communication, and self-care abilities
  • Evaluate the home environment for safety risks and barriers to independent living
  • Develop, implement, and update individualized plans of care in collaboration with the Health Care Provider and RN
  • Complete initial evaluations, treatment notes, and re-evaluations at least every 30 days
  • Complete comprehensive assessments and OASIS documentation accurately and on time
  • Observe, document, and report patient progress and significant changes; obtain updated orders as needed
  • Identify and recommend adaptive equipment and strategies to improve independence with activities of daily living (ADLs)
  • Educate patients and caregivers on adaptive techniques and devices
  • Provide supervision and support to Occupational Therapy Assistants (OTAs), as assigned
  • Collaborate with physicians and interdisciplinary team members to deliver high-quality care
