Transitions At Home, is currently seeking to hire a Part-Time CNA/Home Health Aide in the Spooner area. We’re hiring a dependable and compassionate CNA/HHA to provide hands-on personal care and support to patients in their homes. If you are a caring, compassionate CNA/HHA, competent and perform assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). and appreciates a work family team culture, we would love to add you to our Transitions At Home Team.
What does a Transitions at Home CNA/HHA do?
Transitions At Home CNA/Home Health Aide provides personal cares, bath and hands on care, household services. Reports change in condition and needs to the RN Case Manager in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Qualifications / Job Requirements:
  • Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
  • Must have a current Wisconsin CNA License.

  • Maturity and ability to deal effectively with the demands of the job.
