Job description

Position Title: Registered Nurse
Location: Upland Hills Health - Home Health Services
Role amp; Department: Registered Nurse in the Upland Hills Health Home Health Services Department
Hours amp; Shift: Part-time (.8 FTE) day shift position, Scheduled hours are Monday through Friday 8 a.m. to 4:30 p.m.; rotation of weekends and holidays with on-call responsibilities as well as hospice service call coverage
Role Responsibilities:
  • The Home Health/Hospice Registered Nurse provides skilled nursing care that is individualized for patients in their homes as well as provide call support for home care and hospice patients
  • Collaborate with an interdisciplinary team to support patients and families in both home health and hospice settings
  • Participate in an on-call rotations for evenings, weekends and holidays
  • Maintain accurate and timely documentation according to clinical and regulatory standards
  • Provide patient and caregiver education to promote optimal outcomes
Qualifications:
  • Current Registered Nurse license in the State of Wisconsin
  • Prior Home Health and/or Hospice experience is preferred
  • Prior nursing services experiences are preferred
  • Strong clinical assessment, critical thinking, and organizational skills
  • Ability to work independently while being part of a collaborative team
  • Candidates must have a valid driver’s license and a reliable car available during working hours or accessibility to dependable transportation
  • Must have a good driving record and active Wisconsin Drivers License
  • Must have a caring personality and excellent communication skills
  • Comfortable working on a computer and experience with EMR's is preferred
  • Prior experience with EPIC's EMR is beneficial
  • Previous experience with the Microsoft Office Suite software programs is beneficial
