Position: Home Health Registered Nurse (RN)
Employment Type: Part-Time


Allcare Home Health Agency is a locally owned and operated agency that provides a variety of services to clients in their home and community environment.  Join our compassionate home care team and make a meaningful difference in the lives of patients by providing skilled nursing care in the comfort of their homes


Position Summary

This position offers a combination of office-based responsibilities with in-home field visits, providing an opportunity to build meaningful relationships with patients while delivering high-quality care.

The Home Care Registered Nurse (RN) is responsible for conducting comprehensive patient assessments, developing individualized plans of care, coordinating services with the team, and providing skilled nursing care during home visits.

Responsibilities

  • Perform comprehensive nursing assessments during home visits.
  • Develop, implement, and update individualized care plans.
  • Administer medications, treatments, and wound care as ordered.
  • Educate clients and family on disease management, medications, safety, and self-care.
  • Monitor clients’ progress and identify changes in conditions.
  • Coordinate care with physicians, therapists, social workers, and other healthcare providers.
  • Participate in office-based care coordination, case conferences, and quality improvement activities.
  • Complete accurate and timely clinical documentation         
  • Ensure compliance with agency policies, state regulations, and nursing standards.

Requirements

  • Current, unrestricted Registered Nurse (RN) license.
  • Minimum of four (4) years of clinical nursing experience; home health experience preferred.
  • Excellent assessment, critical thinking, organizational and communication skills.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Technologically savvy, proven ability to learn and master new software and use of variety of tech equipment.
  • Current TB Test
  • BLS Certification (American Heart Association)
  • Valid driver's license, reliable transportation, and current automobile insurance.
