Job description

JOB TITLE: Home Health RN Supervisor / RN Case Manager (RNCM)
  • Work consists of case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as occasional new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
    • Requires back-up for occasional Skilled Nursing visits in most areas.
  • At least 1 year of experience is required in: Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments. Nurse Supervision experience is a plus.
  • At least 1 year of paid experience at a home health agency (or equivalent) is required; 2-5 years' experience is preferred.

SCHEDULE TYPE & SERVICE AREA: Northeast Ohio (2 options)
  • Part-Time RNCM requires a minimum travel radius of Tuscarawas and Coshocton counties plus surrounding areas up to 60-minutes.
  • Full-time RNCM region includes Tuscarawas, Coshocton, Stark, Summit, Cuyahoga, and surrounding counties and areas up to a 2-hour travel radius. Additional cases will be added if more hours are desired (not required).
  • Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled throughout the assigned region.

SCHEDULE TYPE:
  • Flexible and customizable. We'll build a schedule as close to home as possible - that fits your availability and preferences.
  • Minimum availability required is between Monday - Friday (weekdays) from 8am - 5pm.

EMPLOYMENT TYPE: Long-term employment for Full-time or Part-time positions*
  • No On-Call requirements / No Overnight shifts required.
  • Weekend services are optional when available but are not required.
  • Overtime, Respite & PRN services are offered when available but are not required.

Compensation:
  • Part-time and Full-time Hourly Wage: $35 - $40/hour
  • Full-time salary* starts between $72,800 - $83,200 per year, with quarterly bonus opportunities available. *Requires pre-determined minimum visits/services per week.

Job Summary:
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Administrator, or Clinical Administrator/Director of Nursing
Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management required, with home health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
