Job Summary

Job Summary: We are seeking a skilled and dedicated Home Health Admission and Re-Certification Nurse to join our team. The ideal candidate will demonstrate proficient skills using OASIS to admit, transfer, re-certify, resumption, and discharge home health patients in accordance with physician orders. This role involves coordinating delivery of patient care, maintaining compliance with agency policies and procedures, and adhering to CMS regulations.

Key Responsibilities
  • Perform patient admissions, transfers, resumptions, re-certifications, and discharges using OASIS documentation.

  • Initiate and manage individualized plans of care under physician orders.

  • Assess and monitor patient symptoms, reactions, and progress.

  • Educate patients and caregivers on disease processes, medications, and home care techniques.

  • Communicate changes in patient condition to physicians and care teams.

  • Participate in case conferences to ensure coordinated, high-quality care.

  • Maintain accurate and timely clinical records.

  • Stay current with CMS regulations and participate in ongoing education.
