Advanced Home Health and Hospice of Cleveland is hiring a PRN After Hours On-Call RN Nurse Case Manager to provide skilled, compassionate support to patients and families outside of standard business hours. This role is ideal for an experienced RN who is comfortable working independently, managing patient needs in the home, and supporting quality care with professionalism, urgency, and empathy.


This position offers a unique opportunity to combine home health and hospice case management with after-hours responsiveness. We are looking for a nurse who can confidently assess patient needs, provide skilled nursing care, communicate clearly with families and the care team, and help ensure comfort, safety, and continuity of care during evenings, nights, and weekends


Position Details

  • Position: PRN After Hours On-Call RN Nurse Case Manager
  • Location: Advanced Home Health and Hospice of Cleveland
  • Status: PRN
  • Compensation: Paid per visit plus on-call daily pay
  • License Required: Current RN license
  • Experience: At least 1 year preferred

Schedule

  • On-call after hours every 3 weeks
  • Monday through Friday: 5:00 PM-9:00 AM
  • Friday evening 5:00 PM through Monday morning 9:00 AM

Compensation- Flat Per Visist Rates

  • Routine Visit: $70.00
  • Discharge Visit: $80.00
  • Evaluation: $90.00
  • Start of Care: $100.00
  • Resumption of Care: $90.00
  • On-Call Weekday Pay: $25.00 per day
  • On-Call Weekend Pay: $100.00 per day

What We’re Looking For

We are seeking a dependable Registered Nurse who is confident in home health and hospice care, comfortable working autonomously, and committed to delivering patient-centered care in a home-based setting. The ideal candidate is clinically strong, organized, calm under pressure, and able to communicate effectively with patients, families, physicians, and the interdisciplinary team.


Candidates should be comfortable assessing patient needs, coordinating care, supporting comfort and safety across home health and hospice services, and maintaining accurate, timely documentation.

Requirements:
  • Current Registered Nurse (RN) license in the state of practice.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid driver's license and reliable transportation for home visits.
  • Current CPR certification.
  • Experience with electronic medical record systems and documentation preferred.

Physical Requirements

  • Lift and transfer patients and carry equipment or supplies up to 50 pounds
  • Stand, bend, squat, and walk for extended periods during home visits
  • Use fine motor skills for documentation and patient care tasks
  • Respond swiftly and efficiently to urgent or emergency situations
  • Maintain regular attendance and timeliness
  • Work calmly and effectively in high-stress situations
