Job description

Description:

We are currently seeking a Home Health Physical Therapist (PT) to join our growing team. We are hiring for PRN positions. PRN therapists will be paid per visit. As a Physical Therapist, you will provide skilled therapy services in the home setting, focusing on helping patients restore function, improve mobility, and achieve the highest level of independence possible.


Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Deliver skilled therapy interventions in the home environment.
  • Instruct patients and caregivers on therapeutic exercises, mobility, safety, and use of assistive devices.
  • Monitor patient progress, reassess needs, and update care plans as needed.
  • Communicate and collaborate with physicians, nurses, and the interdisciplinary care team.
  • Document care provided in a timely and accurate manner.
  • Travel to patient homes throughout Cincinnati and surrounding areas.
  • Maintain flexibility with scheduling during daytime hours, Monday through Friday.

Requirements:
  • Current, active Physical Therapist license in the State of Ohio (required).

  • Strong clinical assessment, organizational, and communication skills.

  • Ability to work independently and manage a flexible caseload and schedule.
