Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties.ย  It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network.ย  The network works together to create opportunities for older Ohioan's to receive needed home and community services and supports; and to age successfully in their own homes and communities.

Position Overview:ย The role of Community Resource Consultant is to initiate the process of evaluating the individual/consumer needs and providing information regarding available options.ย ย 

Job Duties:

  • Complete face to face assessments (all types) in the community with Consumerย (andย caregiver/contact)ย toย identify unmet needs, determine level of care, and identify health andย safety risks or concerns.ย 
  • Educate consumers, caregivers and stakeholders regarding eligibility of WRAAAย administered programs.
  • Communicate and collaborate withย caregivers, healthcareย and serviceย providers,ย and anyย other formal and informal systems to accuratelyย determineย and meet the individualโ€™sย needs.ย 
  • Provideย options and referrals to meet individualโ€™s needs if not pursuing Medicaid Waiver.ย 
  • Document inย appropriate softwareย systemย according to policies and quality compliance standards.ย 
  • Work cooperatively and constructively with internalย WRAAAย staff, and with formal andย informal systems.ย ย 
  • Advocate on clientโ€™s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization.ย 
  • Collaborate and participate in department and agency initiatives for the purpose ofย improving quality, efficiency, and positive stakeholder interactions.ย 
  • Complete assessments during core business hoursย and occasionallyย early evening orย weekend hours as needed to meet the needs of the community.ย 
  • Adhere to minimum capacity requirements. It is the sole responsibility of the CRC to meetย productivity standards.ย 
  • Participate in training, meetings, best practice committees and LEAN projects.ย 
  • May be asked to provide assistance and back-up to other units within the departmentย and/or other duties as assigned.
  • ย Current licensure as a social worker by the State of Ohio Counselor and Social Work Board, or asย a Registered Nurse with the Ohio Board of Nursing.ย 
  • Two years of experience in home care, geriatrics, home health care, medical social work,ย or other substitute as approved by the Ohio Department of Aging.ย ย 
  • Knowledge of major chronic illnesses, disease processes, normal aging, and basicย knowledge of medical terminology.ย ย 
  • Excellent verbal and written interviewing and assessment skills.ย 
  • Knowledge of local resources for older adults.ย ย 
  • Excellent interpersonal and communication skills to represent our agency in a professional manner and to maintain good working relationships within our agency as well as all external entities and stakeholders.
  • Intermediateย computer skills including Microsoft Office applications.ย ย 
  • Ability to complete job duties by telecommunication from homeย and field.ย 
  • Ability to record information inย an accurate,ย timelyย manner and meet each programโ€™s time frames.ย 
  • Ability to use and will be trainedย inย the Ohio Department of Agingย and Ohio Department ofย Medicaidย documentation systems.ย 
  • Ability to use and transport a laptop computer and case, therefore must be able toย lift/carry a minimum of twenty (20) pounds.ย 
  • Ability to ambulate enough to access private homes/locations which may not be readilyย accessible.ย 
  • Must have a car available during working hours, as well as the ability to legally drive aย car.ย ย
