The Department of Corrections is seeking a highly motivated and qualified Licensed Practical Nurse (LPN4) for a temporary position that is not permanent and part-time at the Monroe Correctional Complex located in Monroe, Washington.
Under the direct or indirect supervision of a Registered Nurse, the Licensed Practical Nurse 4 supports the Department's mission to improve public safety by assisting in the implementation of the nursing process and delivering services to promote health, foster healing, and support coping and adjustment of various health conditions.

Monroe Correctional Complex is one of our larger facilities handing a patient population with many diverse health care needs. Monroe Correctional Complex is our only facility that takes care of dialysis patients in house.
  • 26 bed in patient unit
  • Treatment of chronic disease such as HEP C, diabetes, hypertension, cancer, Alzheimer dementia and COPD
  • Outpatient clinics see 15-20 patients per day

Duties
Assist RN3/IPN with monitoring and enforcing policy for infection Control according to policy 670.000, DOH Health Environment, & safety Standards, MRSA protocol, flu protocol, Policy 590.100, and Policy 890.600
  • Participate in facility Infection Control meetings.
  • Participate in facility audits with safety team
  • Participate in the statewide bi-weekly Infection Prevention Nurse (IPN Meetings)
  • Provide Communicable disease reporting to DOH per WAC 246-101 (-105, -110, -, -120).
  • Monitor and enforce protocols for: MRSA, Flu, Scabies/lice, C-diff, Norovirus and other Gastroenteritis's, Varicella Zoster, and Hepatitis A & B.
  • Monitor and enforce Blood borne Pathogen Protocols through collaboration with Occupational
  • Nurses, offering assistance with source person testing and post exposure management for offenders.
  • Provide outbreak assistance and information as needed.
  • Provide sexually transmitted infection (STll Case Management including education and data management and reporting.
  • Provide notification and education concerning extended family visits.
  • Provide information and education for Offender Immunization Program.
  • Track and manage data on Infection Control including analysis of trends over time and reporting to HQ monthly via the Surveillance Log and other state agencies as required.

Assist RN3/IPN with managing Hepatitis C Program at the facility level to include monitoring adherence to protocols and use of appropriate documentation
  • Track and manage Hep C data including providing reports and information to HQ.
  • Manage patient cases according to protocol and in coordination with providing support to the practitioner.
  • Participate in Hep C CRC Conference Call
  • Participate in Hep C Protocol Committee as designated

Assist with compliance of policy for Isolation Procedures (including signage), teach and educate staff, monitor and enforce compliance according to DOH Health, Environmental, & Safety Standards.
  • Ensure DOC program is in compliance with Personal Protective Equipment (PPE) policy
  • Ensure necessary supplies are available to support procedures. Protect the confidentiality of health records as mandated by law, professional standards, DOC Policy Directives, facility.
  • Operational Memoranda, and Health Care Unit procedures. Treat any and all health records or health information as confidential and privileged.

Assist the RN3/IPN in monitoring and enforcing policies and procedures related to infection prevention and the DOH - Health, Environmental, & Safety Standards for hand washing and sanitation of facilities and supplies
  • Monitor and enforce hand washing.
  • Monitor and enforce sanitation standards for terminal cleaning of living units (showers, kitchens, cells, etc).
  • Monitor and enforce sanitation standards for terminal cleaning of patient rooms
  • Monitor and enforce sanitation standards for the cleaning, sanitizing and/or sterilization of medical and dental equipment and supplies
  • Monitor and enforce sanitation standards for the cleaning, sanitizing and sterilization of barbering tools, equipment and supplies
  • Act as consultant to Safety and Janitorial Services
  • Act as consultant to Laundry Services
  • Act as consultant to Food Services Program
  • Develop and monitor policy for standardization of cleaning products including the use of, and necessity for, bleach

Use tools, techniques, and physical and cognitive skills and abilities to practice nursing safely and effectively in the correctional setting and within the scope of practice defined by state law and administrative code so that patient care provided is safe, effective, efficient, and clinically appropriate
  • Monitor and collect specific data, as directed, about the health status, clinical condition, or situation of individuals, families, groups, and communities by interview, observation, inspection, and examination; Report the results of the data collection to the Registered Nurse
  • Assist the Registered Nurse in developing nursing diagnoses and outcomes to be achieved as a result of nursing intervention.
  • Contribute to the development of the plan of nursing care including identification of aim roaches, timeframes, and pathways for the delivery of health care. Assist the Registered Nurse in prioritizing and sequencing care to be provided to ensure continuity of care over time and place.
  • Carry out the plan of care by performing common therapeutic nursing interventions, informing others about the urgency, time, and methods to deliver health care on and off site. Determine when needs for care or services are beyond the LPN scope of practice, require clarification, or require another type or level of intervention. Take action as necessary to obtain direction from the Registered Nurse and accomplish prescribed interventions in a timely and clinically responsive manner.
  • Assist the Registered Nurse in analysis and evaluation of the structure, process and time frames for service delivery, the expected effects of treatment or intervention, and the outcomes to be achieved
  • Assist the Registered Nurse to revise the plan of care as necessary to prevent complications, to address new or emerging problems or to improve care and service delivery
  • In accordance with the intent and provisions of established policies, practices, and procedures, employ appropriate safety techniques to push, pull, lift, and carry loads

Delegate and assign tasks to other staff as appropriate
  • Supervise and assign tasks to subordinate staff on the nursing team.
  • Delegate and assign responsibility for patient care to other Licensed Practical Nurses and assisting nursing personnel after considering the needs, condition, and stability of the patient, potential for harm, complexity of tasks, and predictability of outcomes.
  • Teach, instruct, monitor, supervise, and evaluate the implementation of tasks and the results of patient care assigned to other personnel
  • Supervise the staff to whom functions have been delegated, and report the outcomes of delegated nursing tasks to the Registered Nurse or other supervising health care provider
Self-evaluate and practice professional nursing conduct at all times
  • Evaluate own practice for technical competency and safety, obtain education or training to retain or improve skills and abilities based upon the work assigned, and inform the immediate supervisor if there are specific gaps in techniques, skills, or abilities.
  • Be in attendance, capable, and competent to practice nursing as assigned. Report for and perform work as scheduled so that staff safety and patient care is not compromised
  • Using established methods, brief appropriate personnel on pertinent information, before departing; from duty, to ensure continuity of patient care. Work overtime (voluntary and mandatory) and callback shifts as necessary to maintain service delivery, safety, and security
Provide effective communication and documentation
  • Effectively communicate verbally, in writing, and via computer or other electronic media with offenders, their families, other health care personnel, custody, and other institutional personnel
  • Legibly document subjective and objective clinical findings and the actions taken to deliver nursing and other aspects of health care in the health record, computerized database, and on other DOC approved forms. All documentation will be in accordance with the Health Care Guidelines.
  • Protect the confidentiality of health records as mandated by law, professional standards, DOC Policy Directives, facility Operational Memoranda, and Health Care Unit procedures. Treat any and all health records or health information as confidential and privileged.
  • Maintain accurate, legible, and complete records of patient care, including chart documentation using appropriate format, of all patient requests for medical attention, objective
  • data collected, treatment administered, and patient education provided. Assist in writing, evaluating, and revising nursing care plans and interdisciplinary treatment plans, and complete other entries to support the problem-oriented health care record.
  • Communicate and assist in the coordination of pertinent information to health care providers for patients scheduled for community-based care
  • In conjunction with the licensed practitioner and the Registered Nurse, coordinate health care received by patients by completing encounter forms for scheduling of needed services in the facility and for on-site community-based services
  • Complete unusual incident reports as directed. Collect data, investigate, and complete reports necessary to the ongoing operation of Health Services and the institution.
  • Respond to verbal and written requests from offenders and institution staff, as directed, providing information or advice consistent with statutes and procedures on health care record confidentiality.
  • Read and respond to email daily.
Administer medication
  • As delegated by the Registered Nurse, responsible for overseeing the medication administration area and patient care in that area.
  • Participate in medication lines and individual medication administration for patients in the clinic and their cells.
  • Process medication orders as written by providers and order medications, including refills, as needed.
  • Investigate misplaced medications and reconcile delivery of packaged medications.
  • Provide patient education regarding medications and prepare release medications in anticipation of offender transfer or release.
  • Adhere to all DOC and Department of Health policies and procedures pertaining to medication administration, including controlled substances, procurement of medications, and reporting to the supervisor medication errors or potential sources for mistakes or mishaps.
Provide competent patient care
  • As delegated by the Registered Nurse, assigned as the lead clinical nurse in the treatment room.
  • Collect subjective and objective data about the physical and mental health conditions of patients during nursing triage, ambulatory care clinics, and inpatient rounds. Report data collected to the Registered Nurse and as directed, initiate appropriate intervention according to Health Services policies and procedures, treatment plans, and protocols.
  • Participate in treatments and dressing changes, monitor vital signs, and assist with minor surgical procedures within the scope of nursing practice and proper training.
  • Evaluate and carry out licensed 12ractitloners' written, verbal, and telephone orders. Assist in checking for allergies, dosages, intended effects of medications, side effects, and drug interactions of medication orders.
  • Ensure that practice is consistent with state licensure laws, administrative codes, policies, and procedures. Report practice deviations to immediate supervisor and participate in corrective action as directed. May assist the supervising RN in the. selection of staff and evaluation of job performance.
  • Assist the Registered Nurse in responding to emergencies. Provide emergency medical care, including CPR in a timely, competent manner
  • Participate in Health Services Quality Improvement activities
  • Practice in accordance with DOC Policy and Offender Health Plan .
  • Participate in formal clinical oversight according to facility and DOC Health Services Clinical Oversight Structure.
