$5000 SignOn Bonus

Under general supervision, this position the Certified Diabetes Educator supports the Joslin Diabetes Center by providing nutritional assessment, education, and management to patients with chronic conditions who are at risk for disease progression, complications, cognitive, and functional decline. In collaboration with the physician and interdisciplinary team, the Certified Diabetes Educator develops and implements an individualized care plan designed to promote the patient's understanding and management of their condition, optimize quality of life, and improve clinical outcomes. This role will also develop various nutritional programs (adult and pediatric focused) for the Joslin Diabetes Center. This position provides service in the Endocrinology office, [and remotely, as appropriate according to schedule and staffing needs.]

Minimum Education, Training, and Experience Required

  • Bachelor's degree with course work approved by the Academy of Nutrition and Dietetics Accreditation Council for Education in Nutrition and Dietetics (ACEND).

  • Certified Diabetes Educator Certification.

  • Passed a national examination administered by the Commission on Dietetic Registration.

  • Possess current, unrestricted certification with the Commission on Dietetics Registration (CDR)

  • Possess current, unrestricted dietitian licensure in Kentucky (Kentucky Department of Professional Licensing) and Indiana (Indiana Professional Licensing Agency)

  • Knowledge of nutrition guidelines in chronic disease management.

  • Knowledge of medical terminology.

  • Knowledge of HEDIS or NCQA quality measures

  • Valid unrestricted driver's license and ability to drive to multiple locations.
