Responsibilities

POSITION SUMMARY: The Group Home Counselor provides supervision, guidance, instruction, and/or skill building to assigned individuals supported within the individual's residence and/or assigned program site regarding daily living activities, health, safety, well-being, vocational activities, and interaction with others.
DUTIES AND RESPONSIBILITIES
  • Manages, supervises, instructs, supports, and provides guidance to assigned individuals served in accordance with the individual's support plan, and works daily toward the designated outcomes while continuously emphasizing community integration in all activities, functions, and aspects of life, including but not limited to:
    • personal healthcare
    • personal hygiene including but not limited to, baths, showers, oral hygiene, personal care, nail and hair grooming
    • religious preferences
    • sexual preferences
    • ethnic and cultural support
    • recreational/leisure/socialization activities
    • physical fitness
    • healthy living
    • family visits and involvement, and family disagreements and grievances
    • independence
    • purchase and inventory of food and personal supplies
    • meal preparation that may include planning the menu, observing any dietary restrictions, shopping for the necessary items, and cooking and serving the meal
    • housekeeping and cleaning which may include bed making, vacuuming, dusting, folding and organizing
    • management of his/her clothing maintenance including but not limited to laundering, mending and ironing
    • money and financial management including but not limited to money recognition, short term and long-term budgeting, banking and bill paying
    • social awareness including but not limited to table manners, telephone skills, and social etiquette
    • interpersonal relationships
    • employment and/or vocational responsibilities, behavior, and performance
    • safety and response to emergencies, including but not limited to fire alarms
  • Performs routine housekeeping duties on a daily basis including sweeping, mopping, dusting, vacuuming and organizing; cleans kitchen appliances as scheduled; cleans and sanitizes bathrooms during each shift
  • Transports individuals to and from program sites, appointments, and community activities including escorting individuals to designated areas
  • Completes laundry, mending, and ironing as assigned
  • Administers all medications within prescribed time frames, inventories medications as required, picks up medications/refills when needed in accordance with agency trained Medication Administration protocols
  • Oversees and supervises the activities and whereabouts of each individual at the level outlined in the Individual Support Plan
  • Purchases groceries based upon menu for upcoming week as assigned
  • Prepares all meals and snacks as outlined on the menu adhering to any dietary restrictions
  • Performs general shopping duties as assigned
  • When assigned to accompany individual(s) supported to a day program, assists with design and implementation of activities; provides guidance, supervision and support while actively engaging with assigned individual(s) through program activities; documents progress toward outcomes specified for day program activities; and completes all required notes, documentation and attendance records
  • Assists the individual with the use, care, maintenance, and repair of prescribed adaptive and medical devices and equipment including but not limited to glasses, hearing aids, gait belts, walkers, wheelchairs, canes, C-pap machines, eating utensils, and communication boards, and ensures that individuals have all prescribed adaptive and medical devices and equipment available to them
  • Provides support to individuals while engaged in community activities which keep them safe, engaged and happy
  • Completes assessments as assigned within agency provided guidelines; participates in planning of supports and behavior management
  • Completes all required documentation assigned, including but not limited to goal training, behavior management, progress notes, communication logs, incident reports, medications, emergencies and repair issues
  • Maintains confidentiality at all times of any sensitive or personal information regarding individuals, employees or Elwyn New Jersey
  • Remains on duty to ensure the care and safety of individuals until relieved by replacement staff
  • Accepts reassignment to support individuals in other residences and locations when and as needed
  • Places trash and recyclables out on scheduled trash days
  • Maintains, organizes, files, purges records as assigned
  • Attends at least eight (8) staff meetings per year
  • Adheres to all current policies and procedures of Elwyn New Jersey
  • Maintains current state-mandated training and certifications
  • Follows all current policies and procedures of Elwyn New Jersey
  • Cooperates with Elwyn New Jersey and the State of New Jersey Department of Human Services, or any other authorized investigating body during an inspection, inquiry, or investigation
  • Performs cardiopulmonary resuscitation (CPR), safe crisis prevention and intervention, and medication administration using agency-trained protocols
  • Performs other duties as assigned
EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:
  • High School Diploma or graduate equivalency diploma (GED)
  • Experience in a related field or with individuals with developmental disabilities preferred
  • Non-profit human service experience highly preferred
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems
  • Must be able to perform CPR, first aid, and safety care crisis prevention and intervention techniques using agency-trained protocols
  • Must possess good customer interaction, collaboration, and written and verbal communication skills
  • Demonstrated basic experience with web-based applications and email applications preferred
  • Must submit to a criminal background check and drug test and be determined to be not disqualified by the New Jersey Department of Human Services prior to working with individuals served
  • Must agree to have their name checked against the New Jersey Central Registry of Offenders Against Individuals with Developmental Disabilities and NJ Child Abuse Registry (CARI).
  • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record.
