Job Specifics
  • Location: 301 E St Joseph St, Green Bay, WI 54301
  • Hours: 20 hours/week
  • FTE Status: 0.50 FTE, Part-Time
  • Work Schedule: Overnight shifts with an every other weekend rotation; Overnight shifts - 11:00pm-7:30am

Job Description:
  • Responsible for case review and telephone communications with patients for assessment, and assistance with appropriate level of care needs and/or safety planning if appropriate.
  • Responsible for answering phone calls, telephone assessment and triage, and scheduling full patient assessments with appropriate level of care resource.
  • Functions as a member of the multi-disciplinary treatment team.
  • Responsible for communications across entire health system related to patient referrals and outcomes.

Qualifications:
  • Bachelor's degree in Social Work, Psychology, Counseling, or related field required.
  • Previous experience in triaging required.
  • Wisconsin Social Work license preferred.
