Part-Time | Monday–Thursday | Approximately 20–24 Hours Per Week

Position Summary

Clearpath Outpatient Clinic LLC is seeking a compassionate and organized Part-Time Case Manager to provide care coordination and support services to individuals receiving outpatient mental health and substance use disorder treatment. This role helps clients overcome barriers to care, connect with community resources, and work toward their recovery and stability goals.

Schedule

  • Part-Time: Approximately 20–24 hours per week
  • Monday–Thursday
  • Weekends off
  • Hours may be scheduled during the clinic's regular operating hours and may offer some flexibility based on client and program needs.

Responsibilities

  • Engage clients using person-centered, trauma-informed approaches.
  • Complete needs assessments and identify barriers to care.
  • Connect clients with housing, benefits, transportation, employment, medical, and recovery resources.
  • Assist clients with referrals, applications, and appointment coordination.
  • Collaborate with the treatment team to support individualized goals.
  • Document services accurately and timely in the electronic health record.
  • Participate in team meetings and supervision.
  • Recognize and escalate safety concerns according to agency policies.

Qualifications
