Job description

JOB TITLE: RN Case Manager / Nursing Supervisor

  • This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.
    • Requires back-up for occasional Skilled Nursing visits in most areas.
  • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required.
    • Must have experience with OASIS and NON-OASIS assessments.
    • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.

SERVICE AREA: Cincinnati, OH + surrounding areas in Hamilton County and adjacent counties

  • Primary service area is centralized throughout Cincinnati, OH + metro areas across Hamilton County, as well as adjacent counties.
  • PRN or Part-Time (up to 20 hrs/wk): Requires typical travel up to a 60-minute radius of Cincinnati, with occasional travel up to 90 minutes also required.
  • Part-Time (21-31 hrs) or Full-Time (32+ hrs): Requires typical travel up to 2-hour radius.

JOB LOCATION OPTIONS:

  • Hamilton County is central to all clients in this service area, but qualified applicants residing outside of but near Cincinnati will be considered so long as travel requirements are fulfilled.

SCHEDULE TYPE:

  • Flexible and customizable. We'll build a schedule as close to home as possible - that fits your availability and preferences.
  • Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).

EMPLOYMENT TYPE:

  • Part-Time or PRN for Long-term* employment within 60-90 minute travel radius.
    • PRN and PT RNCMs receive Hourly Compensation (paid weekly, on Fridays)
  • Part-Time or Full-Time for Long-term* employment within 2-hour travel radius.
    • FT RNCMs have choice between Hourly or Salary Compensation
  • No On-Call requirements / No Overnight shifts required.
  • Weekend, Overtime, Respite & PRN services are offered when available but are not required.

*HCN will consider short-term or temporary employment for those interested in PRN / On-Call / Back-up, but long-term is desired. Please specify on your application answers if you're only interested in this.

  • This is a Part-time or PRN position, both for long-term employment opportunity that can be built to fit your schedule. Typical travel requires up to a 60-minute radius of Cincinnati, with occasional travel up to 90 minutes also required, and offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel).
    • If PRN schedule is preferred, please note that hours and shifts vary week to week, including some weeks having low or no hours at all.
    • Options for Part-Time (21-31 hrs/wk) or Full-time (32+ hrs/wk) are available if desired, if willing to travel up to or beyond 2+ hours as needed.
  • Candidates seeking long-term employment are preferred, however short-term or temporary employment will be considered.

Job Summary
As an RN Case Manager / Nurse Supervisor, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional or Clinical Administrator / Director of Nursing


Responsibilities/Requirements of an RN Case Manager / Clinical Nurse Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
