Position Description:
The Transition Support Permanency Coordinator supports youth preparing to transition from foster care to adulthood, facilitating access to services, education, and community resources that promote self-sufficiency, permanency, and long-term well-being.
Primary Responsibilities:
  • Track all youth turning 14 years old to take the Casey Life Skills Assessment (CLSA) to ensure successful completion within 60 days of being eligible for the assessment.
  • Track youth turning 16 to take the Preparation for Adult Living (PAL) Life Skills Training to ensure successful completion within 60 days of being eligible for the training. If a youth has been unable to complete PAL training by the age of 17, efforts to complete using an independent study guide are considered.
  • Conduct an audit the month before each youth turns 18 to ensure Impact documentation is correct and to avoid any human errors for the measure.
  • Coordinate the provision of all Transitional Living Benefits to youth aging out of care.
  • Assist the youth to get their State Issued ID at the age of 16 and document that in IMPACT.
  • Ensure that you can attend their Circle of Support (COS) and communicate with the provider network to ensure that youth are present; maybe required to transport youth if needed.ย  Ensure that subsequent meetings are scheduled if youth decline.
  • Closely monitor designated mailbox for referrals for Life Skills Training.ย  Communicates with DFPS PAL and DFPS caseworker upon successful completion of training and if there are any barriers in completion.
  • Communicate with DFPS PAL and DFPS caseworker upon successful completion of the Independent Study Guide and if there are any barriers in completion.
  • Educate youth on the benefits of Extended Care, assist youth in signing the Extended Care Agreement and ensure that DFPS PAL and DFPS caseworkers have a copy of the agreement.
  • Confer and plan with DFPS for planned and unplanned discharges from care for youth receiving PAL services.ย  This should include but is not limited to a Circle of Support, connections with family and friends and ensuring youth have all necessary documents.
  • Maintain accurate record of youth discharging from care along with services received and necessary supports provided post discharge.
  • Create and implement strategies to prevent unplanned discharges from paid placements and assist youth with successful transitions from foster care.
Required Qualifications:
  • Bachelorโ€™s degree from an accredited college or university.
  • Minimum two (2) yearsโ€™ experience in the child and family services field.
Preferred Qualifications:
  • Experience working with older youth in Foster Care.ย 
  • Experience with Preparation for Adult Living experience at DFPS.
Knowledge, Skills, and Abilities:
  • Knowledge of child development and the impact of trauma, attachment, and permanency on child and family functioning.
  • Knowledge of DFPS policies, SSCC procedures, and PAL program requirements related to independent living, extended foster care, and transitional services.
  • Ability to assess youth needs and develop individualized transition or life-skills plans that promote permanency and long-term stability.
  • Skilled in effective verbal and written communication, including documentation, correspondence, and case coordination.
  • Skilled in establishing and maintaining effective working relationships with DFPS staff, community partners, service providers, and families.
  • Skilled in problem-solving and critical thinking, including the ability to assess situations and develop appropriate solutions.
  • Ability to operate a personal computer and use standard software applications for case management, documentation, and communication.
  • Strong organizational and time management skills, with the ability to prioritize tasks, manage multiple cases, and meet deadlines.

  • Range of Schedule: Mon โ€“ Fri, 7:00 a.m. to 7:00 p.m.
  • Travel: Frequent โ€“ Local, Statewide, as needed. (Must have reliable personal transportation, valid Texas Driver License and current Auto Insurance)
